The cost of this 38 hour program is $695.00. For those people who are members of Blue Cross - Blue Shield of Western New York and carry the Alive and Lively rider, your only cost would be $145.00. This fee is for the part of the program that is not paid for by the insurance company. The fee is for an individual one hour Lifestyle Assessment consultation. Payments can be made during the program.
